Mistimed - what exactly is it?

I've noticed if you click like a lot of times, it goes to mistimed. For example

Hack, Slash, Mistimed - then you can't continue your combo.

or, Hack, Mistimed. You get the point right?

I know this HAS to happen to you, now what is it? lol

It means you were clicking to fast. There is a certain pace you need to click at in order for the weapon attacks to be affective. If you click to fast it says mistimed and the combo will not continue. If you click to slow the combo doesn't continue.

It means you did not time your combos right. You swing, click, swing, click. Get it?
