Um... one thing that scared me was in Jak 3... I was using a hoverboard and going off this creepy town island of outcasts known as Spargus city, with all the howlings of the desert and slow background theme. It was night, and nights are scary in the game. I took the Board and managed to escape a few meters of the island... When suddenly a LONG tentacle pulls Jak down, with a very scary loud noise.
Same with the Incredibles game. Going with Mr.Incredible off the island of the villain... suddenly huge mines pop out of nowhere and kill me...
POTCO: After the time of the first Muertos Moon, I was telling my friend Leon about Marceline being villains, and that something dark was on the horizon, repeatedly... at the same time the old Padres del Fuego scary music was playing, and me telling the story completely FREAKED myself out!!!!
Few months after the redesigning of Port Royal, when the songs were changed, this song disappeared. Once when privateering I went with my GM, hiding behind a rock. All music stopped, then suddenly the same old single-instrument PDF music was playing, haunting me again and giving me GOOSEBUMPS like anything
Also repeated thinking of Death Omens (when I hadn't seen one) gave the goosebumps...
Apart from that I believe there were many such "instances" with other games....

I discovered that accidentally - don't know if it still works these days...

In Tomb Raider 1 (my very first play through), I had started the third level, The Lost Valley, it didn't look too hard - just a couple of wolves here and there, right? Not too annoying...
Once I climbed that one wall everyone remembers in The Lost Valley, I picked up the object near the skeleton, and dropped down. I expected a few wolves, but instead got Raptors - that totally came unexpected since, well, dinosaurs are extinct. I finished that one, and the one that comes after that. I then continued moving along, when that one song starts, and then the floor starts shaking - then, the T-Rex appears. First time I saw that, here was my reaction:

"...........What..........?"

In Tomb Raider 2, The Great Wall, I was at the end, reading the strategy guide of course, and went down for the last Secret. That large room made it somewhat obvious that something was coming - and of course, it was the T-Rex...
After that one, I picked up the Secret, and left the little hall - when the SECOND T-Rex appeared. I ran back into the small hall....

Tomb Raider 3's The Crash Site has dinosaurs all over the place - this time I saw the T-Rex coming....

There was also the Tomb Of Qualopec in TR1 - the raptors that come out of nowhere kind of startled me....
