I haven't encountered any of those except the first one. I sort of wondered if they did that one on purpose. It really helped us tyros find the place.
One bug that may not be a bug but that does bug me is the reach of the "bad guys." An opponent that is approaching you can hit you from a scale 10 meters or so away, but when I'm at about sword length away and take a swing, I get this comment, "Out of range."
This is particularly galling when I use Graveyard Shackles on a creature and step back, and he hits me anyway. What was the point?
