Anyone seen any of this weirdness?

Okay, so, when helping a friend get grenades, I somehow fell off the wall on the fort at Padres. How bizarre is this?






Then, last night I tried to go to a crewmates ship and found myself in the middle of the ocean. Strange. Today, I was sailing toward what my compass said was Port Royal. This is where I ended up. It's the same island I saw while swimming last night. What is this?
