I've only had this happen in Boss Battles, mainly after sinking the Navy Frigates, or defeating the officer squad, but not with any Voodoo Dolls surprisingly. I've always found this one to be very annoying and I hope Disney adds this glitch to their "List of Glitches to Fix".
