The link is on that same official notification of the game's closure. I clicked on it and saw the same thing but no name written thereon. You can print or save to disk and fill-in your pirate's name.

Bart, sorry for the bother, but how did you get your name on the form, looking all nice and font-appropriate and get rid of the line that was originally there? I'm not overly savvy about such things, but I did save the certificate to my hard drive.
